If your student is beginning school for the first time at a Coffeyville Public School you will need to bring the following with you:

• Birth Certificate
• Social Security Card
• Immunization Record - If your student is between the age of 5 – 8 they will also need to have a health assessment.  Health assessments may be obtained from your local physician, the Montgomery County Health Department or Community Health Center-SEK
